Astigmatism and LASIK Surgery: Is It a Good Option?
Advances in refractive surgery have made it possible to treat many patients with astigmatism more accurately than ever before. However, a thorough evaluation remains the most important step.
LASIK
Dr. Enrique Barragán, Oftalmologo en Monterrey
One of the misconceptions I still hear in my practice is that astigmatism cannot be corrected with LASIK or that laser vision correction is only an option for people with nearsightedness.
It's an understandable belief. Years ago, some patients with certain types or degrees of astigmatism had fewer surgical options. Today, however, advances in diagnostic technology and laser platforms allow us to evaluate and treat many of these cases with greater precision.
That does not mean everyone with astigmatism is a candidate for LASIK, but it does mean that many patients have more options today than they did in the past.
What is astigmatism?
One of the misconceptions I still hear in my practice is that astigmatism cannot be corrected with LASIK or that laser vision correction is only an option for people with nearsightedness.
It's an understandable belief. Years ago, some patients with certain types or degrees of astigmatism had fewer surgical options. Today, however, advances in diagnostic technology and laser platforms allow us to evaluate and treat many of these cases with greater precision.
That does not mean everyone with astigmatism is a candidate for LASIK, but it does mean that many patients have more options today than they did in the past.
How has LASIK changed?
The biggest advances have not been limited to the laser itself.
The way we evaluate patients before surgery has also improved significantly.
Today, detailed corneal imaging allows us to analyze the shape, thickness, and overall health of the cornea with remarkable precision. Tests such as corneal topography and tomography provide essential information that helps us determine whether LASIK is both safe and appropriate for each individual patient.
At the same time, modern laser platforms have become more precise and more personalized than those available years ago.
These advances allow many patients with astigmatism—who may not have been considered candidates in the past—to safely undergo refractive surgery, provided they meet the appropriate clinical criteria.
It's about more than the amount of astigmatism
Patients often ask whether there is a specific level of astigmatism that automatically rules out LASIK.
The answer is no.
The amount of astigmatism is only one part of the evaluation.
We also carefully assess:
The stability of the prescription
Corneal thickness
Corneal shape and regularity
The presence of conditions such as keratoconus
The overall health of the ocular surface
Two patients with similar prescriptions may receive completely different recommendations because every eye is unique.
Why an updated evaluation matters
Occasionally, patients tell me they were informed years ago that they were not candidates for LASIK.
Whenever I hear that, I remind them that ophthalmology continues to evolve.
Diagnostic tools have improved, surgical planning has become more sophisticated, and treatment options have expanded.
That doesn't mean every patient who was previously ineligible can now have surgery—but it does mean that an updated evaluation may provide new answers.
Safety always comes first
The goal of refractive surgery is not simply to perform more procedures.
The goal is to identify which patients are most likely to benefit from surgery safely while maintaining realistic expectations.
Technology continues to improve, but careful clinical judgment remains the most important factor in deciding whether LASIK is the right choice for each individual.
